Support Coordination


Support Coordination is funding offered under the NDIS to assist participants to understand and implement their plans. A Support Coordinator can assist you to creatively source informal, mainstream, community and funded supports. Our Support Coordinator will work with you to successfully engage with providers, maintain appropriate supports and express your choice and control throughout your NDIS journey.

Therapeutic Supports
From Occupational Therapy, Speech Pathology and Psychology to Music Therapy, Hydrotherapy and Art Therapy, HCO will assist you to access effective and appropriate therapy services to help you build and develop skills.

Daily Living Skills
Increase and maintain your ability to live as independently as possible within your own home and the wider community by implementing appropriate supports and Assistive Technology.

Social Support
Increase your community participation whether it be tailored individual support, community interest groups, day programs, social groups or weekend getaways.

Assistance to gain and uphold supported or mainstream forms of employment.

Managing your Plan
HCO’s support Coordinator can support you understand Plan and Self-Management, support with review processes and appeals, navigating the NDIS Participant Portal and implementing all other elements of funding to ensure you have the best opportunities to successfully achieve your goals.

Mainstream Services
Our HCO Support Coordinator has extensive knowledge of a range of mainstream services including education, health, housing, mental health, financial, family support, transport and aged care and can help you connect to them.
